We assess and provide management strategies for complex presentations including Behaviours of Concern and physical ailments commonly associated with developmental disability.<\/p>\n<p>To make a referral, please complete the <a href=\"https:\/\/cddh.monashhealth.org\/wp-content\/uploads\/2023\/12\/cddh-referral-form-2024.pdf\" target=\"_blank\" rel=\"noopener\">CDDH Referral Form<\/a>\u00a0and send it to <a href=\"mailto:CDDH@monashhealth.org\">CDDH@monashhealth.org<\/a>. Please contact our offices on 03 9792 7888 if you require assistance with the form. Once we have the referral, it will be reviewed by our clinical team at their weekly Clinic Review Meeting. Actions taken may include the scheduling of an appointment with one of our Senior Medical Officers, a request for further information or interim clinical advice to the relevant parties.<\/p>\n<p>Please note that as a secondary consultation service, CDDH does not provide acute or emergency care. CDDH does not provide ongoing medical services and ongoing care of the patient remains with the referrer. CDDH does not provide allied health service or psychological assessments.<\/p>\n<p>&nbsp;<\/p>\n<p>REFERRAL CRITERIA<\/p>\n<p>CDDH accepts referrals for patients aged 18 and over, who have an intellectual disability, with or without associated developmental disabilities.<br \/>\nCDDH is a secondary consultation service and does not provide acute or emergency care.<br \/>\nCDDH does not provide on ongoing medical services and ongoing care of the patient remains with the referrer.<br \/>\nCDDH does not provide allied health service or psychological assessments.<\/p>\n<p>THE CONSULTATION<\/p>\n<p>The initial consultation will usually last for 1-1 \u00bd hours and is ideally attended by the patient, a support person with a good knowledge of the person and the presenting problem, and other parties who may be able to contribute.<\/p>\n<p>Within 2-4 weeks, a detailed report, including an assessment and management suggestions, is then sent to the referring doctor and the Medical Treatment Decision Maker, with copies sent to other parties with written consent.<\/p>\n<p>How CDDH respects your privacy: \u00a0<a href=\"https:\/\/cddh.monashhealth.org\/wp-content\/uploads\/2017\/01\/2016-privacy-information-ee.pdf\">CDDH respects your privacy (Easy English)<\/a>[\/vc_column_text][\/vc_column_inner][\/vc_row_inner][vc_row_inner][vc_column_inner][mk_padding_divider][mk_fancy_title color=&#8221;#006bb7&#8243; size=&#8221;16&#8243; font_weight=&#8221;400&#8243; txt_transform=&#8221;uppercase&#8221; font_family=&#8221;none&#8221;]Depression Checklist[\/mk_fancy_title][vc_column_text]People with intellectual disabilities may be unable to describe their feelings or experiences. This contributes to their doctors often finding it difficult to diagnose depression or other disorders of mental health. The Depression Checklist is a tool to assist in this process. The Checklist is designed to be filled out by someone who know the person very well. It guides and records information the doctor can then use to screen for depression or related disorders in adults who are unable to self-report.
